With a large teardrop freshwater pearl and your choice of semi-precious birthstones.
With either sterling silver or 14k gold-fill (or rolled gold), which is 14k gold bonded to a base metal. Gold-fill is hard-wearing and good for allergies.

The freshwater pearl is 9mm in length and the birthstones are approximately 3-4mm in diameter.
